[PHEE-415] Point the test suite at the paymenthub-ee service names - #3

Eighteen hostnames in the integration-test configuration still named
the old ph-ee-* services. Sixteen of them are ${ENV:default} values,
so a deployment could override them, but the defaults were stale and
the suite cannot run outside a cluster that sets every variable.
Three of the renames are not a plain prefix swap: operations-app was
folded into bff and is reached as http://paymenthub-ee-bff:80, the
mojaloop connector dropped its "-java" suffix, and bill-pay is now
paymenthub-ee-p2g.
Two of the values were also malformed, with the port sitting where
svc.cluster.local belongs -- ph-ee-connector-bulk.paymenthub.80.svc
and ph-ee-connector-channel.paymenthub.8443.svc -- so they could not
resolve under any naming. They are now
<service>.paymenthub.svc.cluster.local:<port>.
The CSV fixture named ph-ee-bulk-demo-6.csv is left alone: it is a
real file in the repository, not a service.
Names taken from the operator CRs in mifos-gazelle
(GAZ-348-paymenthub-ee-updates).
