ci(v4): run CI on every pull request - #415

Drop the pull_request paths-ignore (CHANGELOG.md, RELEASE_NOTES*.md, **/*.md) so every PR into v4 reports its required checks; with required checks on, a docs-only PR would otherwise wait on checks that never run. Push filters are unchanged.
